Asset of the Month – January 2020

According to the manufacturer, “The device provides an advanced automated external defibrillator (AED) platform that can be upgraded and configured to meet a wide range of emergency response needs. The all new CPR MAX technology included in the device allows medical directors of emergency medical services (EMS) the flexibility to implement protocols as they evolve, improving patient care.”

Guidance for the Identification of Persons with Suspected 2019 Novel Coronavirus

The Centers for Disease Control and Prevention (CDC) and the Louisiana Office of Public Health continue to closely monitor an outbreak of a 2019 Novel Coronavirus (2019-nCoV) that originated in Wuhan City, Hubei Province, China in December 2019. Coronaviruses are a large family of viruses, most of which cause mild respiratory infection. There are four coronaviruses that are currently circulating in the U.S.: 229E (alpha coronavirus), NL63 (alpha coronavirus), OC43 (beta coronavirus), and HKU1 (beta coronavirus). These coronaviruses are easily detectable though commercial respiratory virus testing. They are distinct from the 2019-nCoV, MERS and SARS. Patients testing positive for any of these endemic coronaviruses should be managed in the same way as patients with other respiratory infections….

LDH Releases Updated EMS Medication Shortage Procedure Per Alliance Request

In response to the current inability of emergency medical transportation services (EMTS) providers to obtain critical and life-saving medications, and in the absence of substitutive alternative or other comparable life-saving medications, the Louisiana Department of Health (LDH), acknowledges that EMTS providers may institute the specific process for the usage of expired critical shortage, pre-hospital medications when good faith efforts are not successful.

United in COVID-19: Louisiana Rural Ambulance Alliance Response

On March 24, 2020, the Louisiana Rural Ambulance Alliance (LRAA) entered into an emergency contract with the state of Louisiana to assist with COVID-19 response. With COVID-19 aggressively spreading throughout the state, units were requested to assist with emergency response in south Louisiana. As LRAA originally deployed 11 units to New Orleans and surrounding areas, that number quickly grew to 20 with the potential to add more. 

Alliance members, including Advanced EMS, Med Express Ambulance Service and Acadian Ambulance Service, rapidly responded to the call for help by sending both Advanced Life Support (ALS) and Basic Life Support (BLS) units to the areas that were most heavily affected. 

The biggest challenges facing providers is the limited access to Personal Protective Equipment (PPE).

 

“The crews are doing great,” explained Rob Daughdril, LAA Emergency Response Coordinator. “Their biggest hurdle is having to wear PPE all the time and being away from home.” Teams are taking the necessary precautions to control the spread of COVID-19 and other illnesses by thoroughly disinfecting ambulance units after each use. “We have a lot of hard surface areas and after every single run we wipe down, mop and disinfect the units,” explained Pam Mackey of Advanced EMS. 

In an act of pure selflessness, well over 60 EMS practitioners are working under the contract. Mackey explained, “I did a lot of soul-searching when I was told we would have to send ambulances.” Mackey, like other providers and medical professionals alike have families and other concerns at home, yet they didn’t hesitate to lend their time and talent to help those in need. 

A pandemic that has affected the entire world, COVID-19 has also brought unity and support for people everywhere. “The crews and providers working this deployment should be greatly commended for their work, dedication and sacrifice,” added Daughdril. “They are the true superheros.”

As the saying goes, “Not all super heros wear capes,” and LRAA members prove it!

Alliance Membership Meeting: March 2020

In 2020, the Louisiana Ambulance Alliance (LAA) is celebrating its 25th Anniversary and the organization hosted this year’s first membership meeting on March 04, 2020, in Woodworth, LA. The organization, comprised of over 60 EMS providers throughout the state of Louisiana, advocates for its members through response, policy and educational opportunities. 

The March 4th meeting was an important stamp in LAA history as it allowed the organization to review the legislative slate for 2020. “LAA advocates for providers because it is part of the organization’s mission and part of why the organization was founded,” explained Curry Landry, LAA Policy Director. “Among other things, LAA is tasked to provide a forum for the exchange and distribution of ideas and information related to the improvement of the emergency medical transport industry and to serve as an advocate for emergency medical transport, promoting improved health status and improvements to the health system for residents of Louisiana.”   

Landry presented the legislative slate to LAA members to gauge and understand their perspectives. Landry added, “It is of the utmost importance to review bills with the members. They have the expertise to know how each and every bill will affect their company and the industry as a whole. To me and the LAA staff, members are the greatest resource available when it comes to legislation during the session. In addition, it is the LAA’s duty to keep its members up-to-date on legal and policy changes that take place at the capitol.”

Membership meetings are key in informing attendees of upcoming events, programs and more. One of which is the CORES Initiative, a program where LAA members can receive assistance with data collection documenting and reporting. “The CORES Initiative meetings provide a wealth of education regarding the understanding of what is requested of the providers when doing their Cost Data Collection and how to better categorize their costs when documenting things during their data collection process,” explained Megan Hedges, LAA Consortium Director. 

Between the legislative slate review and cost reporting information alone, the membership meeting was packed with plenty of resources for members. LAA continues to provide a forum of information exchange and support for providers and looks ahead to a bright future.

CDC Guidance for Providers with Potential COVID-19 Exposure

Coronaviruses are a large family of viruses that are common in humans and in many different species of animals, including camels, cattle, cats, and bats. Rarely, animal coronaviruses can infect people and then spread between people such as with SARS-CoV, MERS-CoV, and now with SARS-CoV-2.

Published and early reports suggest spread from person-to-person most frequently happens during close exposure to a person infected with COVID-19. Person-to-person appears to occur similar to other respiratory viruses, mainly via respiratory droplets produced when an infected person coughs or sneezes. These droplets can land in the mouths, noses, or eyes of people who are nearby or possibly be inhaled into the lungs.  Although not likely to be the predominant mode of transmission, it is not clear the extent to which touching a surface contaminated with the virus and then touching the mouth, nose, or eyes contributes to transmission.

People: Our Greatest Asset

Nearly two months since Louisiana Ambulance Alliance’s (LRAA) deployment to south Louisiana to assist with COVID-19 responses, providers have completed over 2700 transports. The activation of the emergency contract with the state of Louisiana currently has five units (a number that peaked at 20) remaining in south Louisiana from Alliance members including Advanced EMS, Med Express Ambulance Service and Acadian Ambulance Service.

Of all of the resources and assets housed in LRAA and our affiliates, people are our greatest asset! “The disaster responders’ willingness to get the job done with working 16+ hour shifts seven days a week speaks volumes,” explained Rob Daughdril, LAA Emergency Response Coordinator.

LAA/LRAA is a team with many moving parts including the board/staff, members/providers and other stakeholders, who make teamwork the bridge to success in and day out. Providers who are members of the organization do not hesitate to respond and put their best feet forward. “Even with the hazardous environment that they were thrust into, our members do their jobs without hesitation,” added Daughdril. 

By answering the call quickly and carefully, our providers are undoubtedly the best around and heroes of the state of Louisiana. As the state makes strides towards reopening, we want to continue providing support to the providers who serve selflessly every day.

National EMS Week 2020 (May 17 – 23, 2020)

May 17-23, 2020, is the 46th annual National EMS Week. In 1974, President Gerald Ford authorized EMS Week to celebrate EMS practitioners and the important work they do in our nation’s communities.

EMS Week brings together local communities and medical personnel to honor the dedication of those who provide the day-to-day lifesaving services of medicine’s “front line.”  Whether celebrated with a company cookout or a catered lunch; an open house, an awards ceremony or even quiet reflection about what it means to be an EMS practitioner, EMS Week is the perfect time to recognize EMS and all that its practitioners do for our nation.

This year, five days of EMS Week have a theme that represents the diverse nature of EMS.

  • Monday – EMS Education Day

  • Tuesday – Safety Tuesday

  • Wednesday – EMS for Children Day

  • Thursday – Save-A-Life Day (CPR & Stop the Bleed)

  • Friday – EMS Recognition Day

EMS Week: A Symbol of Unity, Pride and Life

Plans for the 2020 National EMS Week began in late 2019, as various organizations representing the EMS industry gathered to brainstorm how to advance events in previous years. The committee was comprised of representatives from the following organizations: Louisiana Ambulance Alliance (LAA), Louisiana Bureau of EMS, Alliance Center of Education for EMS (ACE4EMS), EMS for Children, Louisiana chapters of the Association of Public Safety Communication Officials (APCO) and the National Emergency Number Association (NENA). 

With plans to host events as far north as Shreveport, as far west as Lake Charles and as far south as Mandeville, the committee was expecting the 2020 National EMS Week to not only be the most eventful to ever occur in Louisiana, but to also bring a sense of unity throughout entire state.

Nevertheless, with the 2020 COVID-19 outbreak on the rise, the committee’s plans were exchanged for what would have an even greater impact on Louisiana’s EMS industry, the organizing and distributing of yard signs. LAA, Louisiana Association of Nationally Registered Emergency Medical Technicians (LANREMT) and the Louisiana Bureau of EMS, collaborated to purchase and distribute yard signs penning the words, “A Healthcare Hero Lives Here,” to EMS providers in every part of the state.

In addition, the inaugural White Ribbon Campaign exclaiming #WhiteForLife (credited to LAA Emergency Preparedness Coordinator, Rob Daughdril) was presented as a hopeful annual campaign during National EMS Week to commemorate the EMS providers who serve selflessly each and every day! “The feedback was remarkable and the week was very successful with the yard signs and White Ribbon Campaign, which we hope will continue in future years,” explained Evon Smith, LAA ACE4EMS Director.

Needless to say, while the original plans for EMS Week were exchanged for yard signs, ribbons and a social media campaign, the week represented the unity, pride and sacrifice of EMS providers and gave them well-deserved recognition.

Second Wave of PPE Shortages Expected

There have been increasing requests for PPE to the State’s PPE Cell indicates that our healthcare systems may be experiencing a “second wave” of shortages.  It is important for healthcare partners to continue attempts to procure from routine vendors as much as possible. The State’s PPE cell remains open and receives request via WebEOC but cannot sustain the demands on a large scale as they continue to grow.


 Additional solutions and alternatives are outlined below:

  • The state maintains a current vendor supply list with known vendors indicating available supplies for procurement on the LDH website

  • Examine alternatives such as re-usable PPE when possible. Examples: 3M half face respirators in lieu of disposable masks. Additional items known to have reusable alternatives are surgical gowns and bouffant caps. 

  • Battelle System for decontamination of non-cellulose N-95 masks. While the system is no longer located in Louisiana, the same process is in place and free of charge to hospitals who utilize this system. Shipping process along with packing materials and labels will be supplied to hospitals who register within the Battelle system. Facilities wishing to register should do so through this site linked HERE. Facilities that have already registered will not need to re-register, they will simply need to use their existing site code in the portal.

Effective September 1, 2020 trips previously brokered by National MedTrans for members with United Healthcare of Louisiana coverage will transition to LogistiCare Solutions (https://www.logisticare.com/drive).What does this mean for your office?   

    • Continue to review your National MedTrans provider portal daily to accept or reject any trips for clients who are still brokered through National MedTrans until September 1, 2020.  

    • Please attest to your completed trips immediately after the date of service for transitioned payers so that National MedTrans can process trip payments in a timely manner.

EMS Sleep Health Study Research

Who? Daniel Patterson, PhD, NRP from the University of Pittsburgh Department of Emergency Medicine is leading a research study that seeks to examine the impact of a sleep health and fatigue education and training program tailored to Emergency Medical Services (EMS) clinicians. This research study has financial support from the National Highway Traffic Safety Administration to the National Association of State EMS Officials (NASEMSO). The University of Pittsburgh has partnered with the NASEMSO as a sub-contractor for purposes of conducting this research study.

What? This research study is an experiment that will test the impact of a new sleep health and fatigue training education program. The program is designed to improve the individual EMS clinician’s sleep health and reduce work-related fatigue through education and training. The program will be administered entirely online (via the

internet) and will be accessible to EMS clinicians located at the EMS agencies that agree to participate in this research study.

When? The study team will begin recruiting EMS agencies to participate in this study in late January 2020. Each agency will be asked to participate for a total of 24 weeks. Participation is voluntary.

How? Researchers will ask EMS agency administrators to help recruit individual EMS clinicians at their agency to participate in this research study. Participation will be completely voluntary and confidential. The study team will mostly use data collection tools available via the Internet. Some data collection will involve mobile phone text messages. The research study’s website will be secure and require a unique login (username and password) from each individual EMS clinician. Some EMS clinicians may be asked to wear a wrist actigraph to measure sleep and complete a reaction time test at the start and end of a few scheduled work shifts during the study period.

Who is Eligible? The study team is seeking participation from EMS agencies located in the United States (including Alaska and Hawaii). Criteria for eligibility include: [1] The EMS agency provides EMS services (including 911 response and transport). [2] The EMS agency provides ground-based EMS services 24-hours a day. Agencies limited to air-medical services only are not eligible. [3] The EMS agency employs between 50 and 300 EMS paid full-time and part-time clinicians/personnel. Agencies that use an all-volunteer staffing model are not eligible. [4] Agencies restrict their EMS clinicians to use their personal mobile phones/smartphones during shifts are not eligible. [5] Operations that provide both fire suppression and EMS 911 response and transport are eligible and encouraged to participate.

Remuneration: Those who qualify for the study and choose to participate will receive remuneration worth approximately $35 U.S. dollars. All individual participants will receive remuneration in the form of a gift card totaling approximately $35 in value. A $5 gift card will be distributed at the beginning, when the individual enrolls, every month the individual is involved in the study, and at the end of the study (month 6). All gift cards will be distributed via U.S. Mail directly to individual participants.

‘Nightwatch” Returning to New Orleans

The A&E Network’s documentary series “Nightwatch”, which chronicles first responders on the job, will return to film a fourth season set in New Orleans after a three-year hiatus from the city, officials announced Friday.

Filming for the 25-episode season focusing on New Orleans Emergency Medical Services’ paramedics who work 12-hour overnight shifts began earlier this week and should continue into next year, said a statement from the agency’s director, Dr. Emily Nichols.

Nichols’ statement said it was “an honor” for the city’s paramedics to be featured before a national audience during a year in which they “have given so much of themselves,” doing their work amid the ongoing coronavirus pandemic.

“We are thrilled to … showcase the beauty and resilience of New Orleans,” Nichols said.

Produced by the companies 44 Blue Productions and Dick Wolf Reality, “Nightwatch” aired three seasons set in New Orleans between 2015 and 2017, shining a spotlight on local emergency responders.

But the city ended filming arrangements with both “Nightwatch” and “The First 48” — which chronicled homicide investigations across the country — after a murder suspect’s attorneys accused producers of “The First 48” of withholding footage that could benefit the defendant. The “First 48” is also shown on the A&E Network.

The production company Kirkstall Road Enterprises swore in court that it had no extra tape. But the suspect’s attorneys later turned up footage of an interview with a friend of the murder victim, prompting the judge overseeing the case to criticize city officials’ decision to let “The First 48” embed with the police.

The executive producer of “Nightwatch,” Rasha Drachkovitch, thanked Mayor LaToya Cantrell for greenlighting the return of the show to New Orleans.

“We are excited to once again tell the stories of these heroes,” Drachkovitch said.

Pafford EMS Deliver Baby

It’s not every day that first responders gets to assist in and experience a miracle as special as bringing a sweet baby into the world!
Recently, Seth Hostetler (Paramedic) and Shane Huggins (EMT) who helped deliver Baby Cade stopped by the family’s home for a visit. Cade’s Grandmother, Terrica Hendrix, shared, “The Pafford EMTs who delivered Cade stopped by our home to visit him this morning. I thank them both so much for all they’ve done for my grandson and my daughter, Skylar Hendrix. This is a sacred bond between Cade Rylan, Jalyn, and the paramedics. The EMTs are Seth Hostetler and Shane Huggins. They are incredible people who ensured and kept my two precious babies safe.”

What a blessing to be able to share in such a special moment and a great story for Mom and Baby Cade to tell for years to come! Congratulations!

LAA Tackles Hurricane Laura

In the midst of the 2020 Pandemic, Louisiana was facing not one but two storms brewing in the gulf coast during the week of August 20th. While Tropical Storm Marco fizzled out before landfall, Hurricane Laura was coming in strong. Louisiana began to prep for a category 4 hurricane. In preparation for Laura, Louisiana Ambulance Alliance gathered over 25 ambulances from across the state to send to Lake Charles to help evacuate the city before Laura made landfall. 

On August 27, 2020 Hurricane Laura made landfall as one of the strongest hurricanes in the last 150 years. While the usual water damage was not a problem, Lake Charles faced devastating wind damage, leaving thousands without electricity, clean water, or even a roof over their heads. LAA sent another 25+ ambulances to Lake Charles to help with the aftermath and hurricane relief effort. Our crews have been stationed in Lake Charles for two weeks now and ongoing.

Phase 3 Provider Relief Funds

October 1, 2020 – under the leadership of President Trump, the U.S. Department of Health and Human Services (HHS), through the Health Resources and Services Administration (HRSA), announced $20 billion in new funding for providers on the frontlines of the coronavirus pandemic. Under this Phase 3 General Distribution allocation, providers that have already received Provider Relief Fund payments will be invited to apply for additional funding that considers financial losses and changes in operating expenses caused by the coronavirus. Previously ineligible providers, such as those who began practicing in 2020 will also be invited to apply, and an expanded group of behavioral health providers confronting the emergence of increased mental health and substance use issues exacerbated by the pandemic will also be eligible for relief payments.

Application window begins October 5, 2020 and will end on November 6, 2020.

Asset of the Month – December 2020

The December Asset of the Month is the Satellite Communications System, including a TracStar .96 meter auto-acquireantenna, an iDirect 3100 satellite modem, a TracStarmeter auto-acquire antenna, an iDirect 3100 satellite modem, a TracStar controller, a Linksys ATA to convert analog phone lines to IP. The Satellite Communications system is used when installed on Disaster Command Trailers.
